Medical Documents

Qualified Applicants:

  1. Patient aged 18 or above (For juvenile, legal guardian should act on behalf); or
  2. Individual or entity being written authorized by patient; or
  3. Legal successor of passed-away patient; or
  4. Individual with court order to manage patient’s properties.

Documents Required

  1. Completed “Application Form for Medical Documents”;
  2. The original and A4 copy of applicant’s identity document;
  3. Extra documents for 2nd person application:

    Juvenile's Guardian
    Individual or entity being written authorized by patient Legal successor of passed-away patient
    • Dealth Certificate
    • Successor's Proof
    • Identity document of both
    Individual with court order to manage patient’s properties
    • Certificate issued by Court
    • Identity document of both

*Remarks: Please submit A4 identity document copy and show original for verification.

 

Processing Period: 

  1. Medical Record Copy:
    • Standard Apply – within 10 working days;
    • Express Apply – within 3 working days;
  2. Other medical documents (For example: Medical Certificate/Insurance Claim Form/Medical Report) – within 30 working days;
  3. More than 30 days may require under for the following conditions:
    • As specially requested by doctor due to its complexity;
    • Multiple document application;
    • Document needs to be written by specialist.

Pick-up Method:

When the document is ready for pick-up, an email or SMS will be sent to the applicant who has to collect it within 90 days at the hospital. After this period, the unclaimed document will be forfeited without prior notice. The re-issuance of the document will require a new application and payment.
